Fire Department of the Year Application
Download the NSFA Fire Department of the Year form as a .pdf file (22.7 kb). The NSFA Board of Directors will select an applicant during the Spring NSFA Meeting. The Fire Department of the Year Award will be presented at the annual conference.

Firefighter Memorial Form
Download the NSFA Firefighter Memorial form as a .pdf (10.7 kb) or as a .doc file (20.5 kb). The Nevada State Firefighters' Association annually commemorates and honors the spirit of association members who passed away during the preceding year with a Memorial Service.

Instructor Excellence Nomination
Download the Instructor of The Year form as a .pdf (14.4 kb) or as a .doc file (203 kb). The Dave Drew Instructor Excellence Award nomination form must be returned by April 15th. The past recipients of this award will select an applicant after the spring NSFA meeting. The award will be presented at the annual conference. (Read more about this award.)

NSFA Membership Application FY2007-08
Download the NSFA Membership form as a .pdf file (36 kb). NSFA membership runs from July 1st of each year to June 30th of the next year.

Scholarship Application
Download the Scholarship Application form as a .pdf (9.82 kb) or as a .doc file (22.5 kb). The Nevada State Firefighters’ Association Scholarship is open to all sons and daughters of eligible members of the Nevada State Firefighters' Association. The association grants a maximum of five $1,000.00 scholarships.

Service Award Program
Download the non-technical explanation of the Service Award Program rules (as of 2007.) as a .pdf file or as a .doc file. Volunteer fire companies located in the state of Nevada may adopt the NSFA Service Awards program.

Service Pin Award Application
Download the Service Pin Award application form as a .pdf or as a .doc file. Service Pin Awards are available to any eligible Association member upon submission of this application by the affiliate department. Service pins and/or certificates will be issued for lengths of service. (See form for details.)
